Resumen Introducción: La pandemia de COVID-19 ha afectado a millones de personas en todo el mundo. La identificación de sujetos infectados ha sido importante para el control. Objetivo: Evaluar el rendimiento de una reacción de polimerasa en cadena (RPC) cuantitativa en tiempo real (en inglés: RT-qPCR) para SARS-CoV-2, utilizando saliva como matriz en comparación con un hisopado nasofaríngeo (HNF). Metodología: Se reclutaron adultos en atención ambulatoria, la mayoría sintomáticos. Fueron estudiadas 530 muestras pareadas de saliva e HNF con RT-qPCR. Resultados: Fueron positivas 59 muestras de HNF y 54 de saliva. La sensibilidad con saliva fue 91%, especificidad 100%, el valor predictor positivo (VPP) 100%, valor predictor negativo (VPN) 98%. El índice Kappa fue de 0,95 y LR-0,08. En promedio, el umbral de ciclo (en inglés cycle threshold-CT) de la saliva fue 3,99 puntos más alto que los de HNF (p < 0,0001) mostrando que la carga viral (CV) es menor en saliva. La carga viral en ambas disminuyó con el tiempo después del inicio de los síntomas. El muestreo de saliva fue preferido por los sujetos en lugar de HNF. Conclusión: Este estudio demuestra que la RPC para SARS-CoV-2 utilizando saliva, es adecuada para el diagnóstico de COVID-19 en adultos ambulatorios, especialmente en la etapa temprana de los síntomas.


Abstract Background: The COVID-19 pandemic has affected millions of people around the world. Part of control strategies is testing a large proportion of the population to identify and isolate the infected subjects. Aim: To evaluate the SARS-CoV-2 detection by the performance of a reverse transcription and quantitative polymerase chain reaction (RT-qPCR) against SARS-CoV-2, using saliva as a matrix compared to a nasopharyngeal swab (NPS) to simplify obtaining a diagnostic sample. Methods: Adults in outpatient care were recruited, 95% of them symptomatic. We studied 530 paired saliva and NPS samples by SARS-CoV-2 RT-qPCR. Results: Fifty-nine individuals tested positive in NPS and 54 in saliva samples. Sensitivity for saliva sample was 91%, specificity 100%, positive predictive value (PPV) 100%, negative predictive value (NPV) 98%. The Kappa index was 0.95 and LR-0.08. On average, the cycle threshold (CT) of saliva was 3.99 points higher than those of NPS (p < 0.0001) showing that viral load (VL) is lower in saliva than in NPS. Viral load in both decreased over the time after onset of symptoms. Saliva sampling was preferred by subjects instead of NPS. Conclusion: This study demonstrates that SARS-CoV-2 RT-qPCR using saliva, even with lower VL, is suitable for the diagnosis of COVID-19 in outpatient adults, especially at early stage of symptoms.

INTRODUCCIÓN: El principal rol de la vitamina D es la regulación del metabolismo del calcio, cuya principal fuen te es la vitamina D3 que se obtiene principalmente por la acción de la luz ultravioleta (UV) en la piel. OBJETIVO: Evaluar las diferencias estacionales en las concentraciones de 25-hidroxi-vitamina D3 (25OHVitD3), hormona paratiroidea (PTH), fosfatasa alcalina (FA) y calcio en niños en edad esco lar. SUJETOS Y MÉTODO: Se midieron las concentraciones de 25OHVitD3, PTH, FA y calcio en niños de 5 a 8 años, sin suplementación de Vitamina D, reclutados en Santiago de Chile (latitud -33.4372) en distintas estaciones del año. El estatus de VitD fue definido como suficiente con concentraciones de 25OHVitD3 > 20 ng/mL (50 nmol/L), insuficiente 12-20 ng/mL (30-50 nmol/L) y deficiente 20 ng/mL) en verano, lo que disminuyó significativamente en invierno (54,3%, p < 0,0001). CONCLUSIONES: Las concentraciones de 25OHVitD3 disminuyeron en aproximadamente la mitad de los niños durante el invierno, lo que se vio acompañado de un aumento de la PTH y FA, asociado a concentraciones normales de calcio. De acuerdo a nuestros resultados, la suplementación con VitD en niños podría ser necesaria durante otoño e invierno.


INTRODUCTION: The main role of Vitamin D is to regulate calcium metabolism, whose main source is vitamin D3 ob tained mostly from the action of ultraviolet (UV) light on the skin. OBJECTIVE: To evaluate the seaso nal differences in the concentrations of 25-hydroxy-vitamin D3 (25OHVitD3), parathyroid hormone (PTH), alkaline phosphatase (ALP), and calcium in school-age children. SUBJECTS AND METHOD: The concentrations of 25OHVitD3, PTH, ALP, and calcium were measured in children from Santiago, Chile (latitude -33.4372), aged 5 to 8 years, without Vitamin D supplementation, in different seasons of the year. VitD status was defined as sufficient with concentrations of 25OHVitD3 >20 ng/mL (50 nmol/L), insufficient 12-20 ng/mL (30-50 nmol/L) and deficient 20 ng/mL), which decreased significantly in winter to 54.3% (p <0.0001). CONCLUSIONS: In winter, 25OHVitD3 concentrations decreased in approximately half of the children, which was associated with an increase in PTH and ALP, and normal calcium concentrations. According to our results, children may need VitD supple mentation during fall and winter.

ABSTRACT

Resumen Los centros de información y asesoramiento toxicológico CIATs de América Latina, en el contexto de la pandemia de COVID-19, recibieron una serie de llamadas para consultas y asesoramientos relacionados con el uso de dióxido de cloro/clorito de sodio, que se estaban empleando en el tratamiento o prevención de dicha enfermedad. Dentro de la legislación vigente en los países de América Latina, no se contemplan productos farmacéuticos registrados para uso en humanos, ni se tiene evidencia de registros sanitarios en Europa, Canadá o Estados Unidos para tal fin, que contengan dióxido de cloro o clorito de sodio. Esta publicación, compila la información registrada como parte de la estadística del trabajo de ocho CIATs correspondientes a igual número de países de América Latina. Se identificó sexo, edad, sintomatología, circunstancia y grado de severidad de los 56 casos de pacientes intoxicados con dióxido de cloro/clorito de sodio registrados en el período del 15 de marzo al 30 de septiembre de 2020 en estos ocho países. Los resultados obtenidos confirman que la causa más común fue por mal uso, y el lugar de ocurrencia fue el hogar o sus alrededores, siendo el mayor porcentaje adultos jóvenes comprendidos entre 30 y 49 años. Los síntomas de intoxicación más frecuentemente encontrados fueron gastrointestinales, seguidos de cardiovasculares y respiratorios. La vía de ingreso al organismo en la mayoría de los casos fue por vía oral, reportándose algunos casos por vía inhalatoria, y en el 50% de los casos se constituyeron casos de severidad moderada, severa o fatal (3 fallecimientos). Este estudio contribuye a generar información relevante para las diferentes autoridades sanitarias, los ministerios de salud, las entidades encargadas de inspección, vigilancia y control de los países en los que se comercializan estos productos de manera ilegal por medio de redes sociales y promoviéndolos para uso en humanos para prevenir o curar COVID-19.


Abstract The Poison Control Centers in Latin America, in the context of COVID-19 pandemic, received a series of calls for consultations and recommendations related to the use of chlorine dioxide/sodium chlorite, in the treatment or prevention of CO-VID-19. Under current legislation in Latin America, no pharmaceutical products are registered for use in humans that contain chlorine dioxide or sodium chlorite, nor is there evidence of sanitary registries in Europe, Canada, or the United States for this purpose. This publication compiles the information registered by eight Poison Control Centers that correspond to the same number of Latin American countries. Sex, age, symptoms, circumstance, and degree of severity of the 56 cases of patients poisoned with chlorine dioxide/ sodium chlorite registered in the period from March 15th to September 30th, 2020 were identified. The results obtained confirm that the most common cause of poisoning was unintentional misuse, all of which occurred at home or its surroundings, with the highest percentage of registered cases being young adults between 30 and 49 years old. The most frequent symptoms of intoxication were gastrointestinal, followed by cardiovascular and respiratory. The route of exposure in most cases was oral, with some cases reported by inhalation; 48.2% of the cases were of moderate, severe, or fatal (3 deaths). This study contributes to the generation of relevant information for different health authorities, ministries of health, entities in charge of inspection, surveillance, and control in countries where these products are illegally marketed through social networks and promoted for use in humans to prevent or cure COVID-19.

ABSTRACT

Background The consumption of illicit drugs in Chile has increased over the years generating abuse and dependence problems and becoming a public health problem. Aim To build and disseminate a figure in which health personnel can quickly visualize illicit drugs and their common names, and thus be able to associate them with the effects and risks that are expected in the patient. Material and Methods A bibliographic review and compilation of information obtained from dealers and drug abusers. Results A figure and a table were developed. The former illustrates the classification and common names of drugs, while the second indicates the mechanisms of action and effects expected in the body according to each drug of abuse. Conclusions Illicit drugs have several popular names and various mechanisms of action. When confronting drug consumption this information is crucial to provide an adequate treatment and withdrawal management. However, it is important to keep in mind that this work only represents a management guide and that treatment should always focus on the condition and clinical manifestations of the patient.

ABSTRACT

About 15% of the essential hypertensive patients would have a low activity of the 11ßHSD2 enzyme, which inactivates cortisol (F) to cortisone (E). Gene expression can be negatively regulated by miRNA. Urinary exosomes and their specific content (miRNA/proteins) represent a valuable tool as a biomarker for the diagnosis and prognosis of the disease. Aim: To evaluate the expression of miRNA specific for 11ßHSD2 in samples of urinary exosomes and to determine its association with biochemical variables associated with mineralocorticoid metabolism. Subjects and Methods: Cross-sectional study in subjects between 10-60 years. They were classified into subjects with high F/E (> p75) and low cortisone (< p25) and control subjects. The urinary exosomes were isolated with the Invitrogen kit. Bioinformatic analysis was performed with Mir Walk to identify specific miRNAs of HSD11B2. The expression of miRNA was evaluated by qRT PCR. The comparisons were made with the Mann-Whitney test. Results: 7.1% of the subjects are suggestive of a partial deficiency of 11ßHSD2 (NC-AME). The expression of miR-488 was higher in NC-AME than in controls (5839 ± 1719 vs 3,437 ± 2,581; p = 0.01). We found positive associations between mir-615 and ARP; miR-488 and the sodium/potassium ratio; miR-1205 with age and urinary sodium excretion; miR-494 with age, activity MMP9 and NGAL. Conclusion: We identified high expression of miR488 in NC-AME subjects and associations of miRNAs with biochemical variables associated with mineralocorticoid metabolism. Thus, exosomes and their miRNA content could be potential regulators and biomarkers of 11ßHSD2 activity.

ABSTRACT

INTRODUCCIÓN: las manos poseen un papel importante en la transmisión de infecciones en instituciones de salud, entornos industriales como la industria alimentaria y en toda la comunidad, por lo que su higiene no puede pasar desapercibida. Los jabones líquidos son actualmente una herramienta fundamental para la limpieza de las manos y así también evitar la transmisión de agentes patógenos. OBJETIVO: evaluar la efectividad del sulfato de cobre como agente antiséptico en un jabón líquido y caracterizar el producto mediante controles de calidad. MÉTODOS: se evaluó la capacidad antiséptica del jabón líquido mediante el método de dilución en agar y la disminución del crecimiento bacteriano a través de una variación del recuento por microgota, a tiempos 1, 5 y 10 min. Posteriormente se realizaron controles fisicoquímicos, microbiológicos, prueba de parche y estudios de estabilidad en estantería y acelerada durante 6 meses. RESULTADOS: las cepas Staphylococcus aureus, Staphyloccus epidermidis, Escherichia coli, Pseudomona aeruginosay Klebsiella pneumoniae fueron sensibles a una concentración de 0,5 por ciento de sulfato de cobre. El crecimiento bacteriano en las cepas S. aureus y S. epidermidis mostró una reducción considerable en los tiempos 5 y 10 min a concentraciones de 0,3 y 0,5 por ciento de sulfato de cobre. El producto elaborado cumplió con los criterios de calidad establecidos, el cual mantuvo su estabilidad durante el tiempo de estudio. CONCLUSIONES: es posible elaborar un jabón líquido en base a sulfato de cobre como agente antiséptico, al ser eficaz frente a las cepas estudiadas(AU)


INTRODUCTION: the hands play a significant role in the transmission of infections at health institutions, industrial environments such as the food industry and at the community; therefore, hand hygiene cannot be ignored. Liquid soaps are now a fundamental tool for hand cleaning and prevention of pathogen transmission. OBJECTIVE: to evaluate the effectiveness of copper sulfate as an antiseptic agent in a liquid soap and to characterize the product by using quality controls. METHODS: aliquid soap was developed, in which its antiseptic capacity was evaluated through the agar dilution method and the reduction of bacterial growth by using a variant of microdrop counting at 1, 5 and 10 minutes. Then physicochemical and microbiological controls were performed, as well as patch test and shelf life and accelerated stability studies for six months. RESULTS: Staphylococcus aureus, Staphyloccus epidermidis, Escherichia coli, Pseudomona aeruginosa and Klebsiella pneumoniae were susceptible to 0.5 percent copper sulfate concentration. Bacterial growth showed considerable reduction in S. aureus and S. epidermidis strains at 5 and 10 minutes at copper sulphate concentrations of 0.3 and 0.5 percent. The finished product met the quality criteria for this type of product, maintaining its stability during the study period. CONCLUSIONS: it is possible to produce a copper sulfate-based liquid soap as an antiseptic since it was effective to control the studied strains(AU)

Background: The determination ofthyroid stimulating hormone (TSH) reference values is critical for the diagnosis ofthyroid diseases. Aim: To explore and discuss different definitions to establish TSH reference values using a Chilean national survey sample. Material and Methods: The 2009-2010 Chilean National Health Survey recruited 5,416participants between the ages of 15 and 96years, from all geographic regions of Chile, including urban and rural zones. TSH was measured in a random subsample of 2,785 adults. Median value, 2.5 and 97.5 percentiles were described in three different populations: total survey population, "disease-free population" and the "laboratory kit disease free population". Results: TSH values were higher among women, the elderly and the less educated population. The 97.5 percentile value in the disease-free population was 7.46 uUl/ml. Using this value as a cut-off, hypothyroidism prevalence would be 4.8% in Chile and estimated pharmacological treatment coverage would be 58%. When laboratory kit cut-offs are used, prevalence rises to 22% and treatment coverage drops to 12%. The 2.5 percentile value in the disease-free population was 0.83 uUl/ml, which yields an estimated hyperthyroidism prevalence of3.89%. Conclusions: Median TSH concentration values in the Chilean "disease-free population" are higher than those proposed by laboratory kits and those of developed countries. TSH values in the general population of Chile are also higher in women, the elderly and the less educated population.

Background: Thyroid hormones play an important role in fetal neural and cognitive development. Therefore thyroid abnormalities should be detected and treated early during pregnancy. Aim: To assess the frequency and risk factors for functional thyroid disorders during the first trimester of pregnancy. Material and Methods: A blood sample was obtained from women during their first trimester of pregnancy, consulting in a prenatal care facility. Women with known thyroid diseases were excluded from the study. Thyroid stimulating hormone (TSH), total thyroxine (T4) and free thyroxine (fT4) were measured by electrochemoluminiscence. Antithyroid peroxidase antibodies (anti TPO) were measured by enzyme immunoassay. Results: Five hundred and ten women aged 25.7 ± 6.6 years were assessed. The frequency of clinical hypothyroidism was 0.6%, subclinical hypothyroidism 35.3% and clinical hyperthyroidism 1%. Five percent of women with hypothyroidism and 3.5% of euthyroid women had positive anti TPO antibodies. There was no association between the frequency of thyroid diseases and risk factors for thyroid diseases. Conclusions: There is a high frequency of subclinical thyroid diseases among women consulting in this prenatal care clinic.

Background: Thiopurines (azathioprine and 6-mercaptopurine) are highly effective medications but with potential adverse effects. Thiopurine methyltransferase (TMPT) is the key enzyme in their pharmacokinetics and is genetically regulated. A low activity of TPMT is associated with myelotoxicity. The genotype and enzyme activity can vary by ethnicity. Aim: To study the activity and genotype of TPMT in a group of Chilean subjects. Material and Methods: In 200 healthy adult blood donors, TPMT activity was determined by high performance liquid chromatography (HPLC). Deficient, low, normal or high levels were defined when enzymatic activity was < 5, 6-24,25-55 and > 56 nmol/grHb/h, respectively. Genotyping of TPMT (*1, *2, *3A, *3B, *3C) was performed by PCR. Results: Seventy seven women (38.5%) and 123 men (61.5%), with an average age of 34.9 years were studied. Eighteen subjects (9%) had a low enzymatic activity, 178 (89%) had normal activity, 4 (2%) had high activity and no genotype deficient subjects were identified. The wild type genotype (*1) was found in 184 (92%) individuals and 16 (8%) were heterozygous for the variants: *2 (n = 2), *3A (n = 13) and *3C (n = 1). No homozygous subjects for these variants were identified. Wild type genotype had an increased enzymatic activity (40.8 ± 7.2 nmol/gHb/h) compared to heterozygous group (21.2 ± 3 nmol/ gHb/h; p < 0.001). Conclusions: Less than 10% of a Chilean population sample has a low enzymatic activity or allelic variants in the TPMT gene, supporting the use of thiopurines according to international recommendations.

ABSTRACT

Bacterial identification is important for the proper treatment of infected patients hospitalized with serious infections especially in critical care units. Identification by conventional methods used in microbiology laboratories takes at least 16 hours since a culture is positive. The introduction of mass spectrometry, specifically MALDI-TOF MS (matrix-assisted laser desorption/ ionization time-of-flight mass spectrometer) in the microbiology laboratory could mean a radical change in the identification accuracy, turn around time (6 minutes per bacteria) and cost (about 5 times cheaper than conventional identification). Since its introduction in clinical microbiology laboratories in 2008, many reports about its usefulness in identifying microorganisms from colonies, as well as directly from positive blood cultures and urine samples have been published. This review describes MALDI-TOF MS methodology, its identification performance for bacteria (aerobic and anaerobic), mycobacterium and yeasts, its future applications in microbiology and its main disadvantages.


La identificación bacteriana es muy importante en el manejo adecuado de los pacientes infectados, especialmente aquellos con infecciones graves hospitalizados en unidades de pacientes críticos. La identificación por los métodos convencionales utilizados en los laboratorios de microbiología clínica demora al menos 16 horas desde que un cultivo es positivo. La introducción de la espectrometría de masas, específicamente del espectrómetro de masas por tiempo de migración (tiempo de vuelo) con desorción/ionización laser asistida por una matriz (MALDI-TOF MS, por su sigla en inglés matrix-assisted laser desorption/ionization time-of-flight mass spectrometer), en el laboratorio de microbiología podría significar un cambio radical en la precisión de la identificación, el tiempo de detección (6 minutos por bacterias) y el costo (aproximadamente 5 veces más económico que la identificación convencional). Desde su introducción en los laboratorios de microbiología clínica en el año 2008, se han escrito numerosas publicaciones sobre su utilidad en la identificación de microorganismos desde colonias, así como directamente desde hemocultivos positivos y de muestras de orina. Esta revisión describe la metodología de MALDI-TOF MS, su rendimiento en la identificación de bacterias aerobias, anaerobias, micobacterias y levaduras, sus futuras aplicaciones en microbiología y sus principales desventajas.

It has been demonstrated that HLA-B*5701 screening reduces the risk for hypersensitivity reaction to abacavir in HIV-infected patients. Since B*5701 prevalence varies among different populations, it is important to determine the carrier frequency prior to its use for the screening of HIV-infected patients.The aim of this study was to determine HLA-B*5701 carrier frequency in Chilean general population and HIV-infected patients referred for B*5701 typing. For that purpose 300 blood bank donors and 492 abacavir-naïve HIV-infected patients from Chile were screened for B*5701 by a sequence specific primer PCR.We detected 14/300 (4.7 percent) B*57-positive individuals in the Chilean general population, 11 (3.7 percent) were B*5701 positive, and 3 (1 percent) had another subtype.All were heterozygous,thus a B*5701 allele frequency of 2 percent was determined.Eleven of 492 (2.2 percent) HIV-patients carried a B*5701 allele. The difference between these frequencies is probably due to slow progression of HIV infection in HLA-B*5701 carriers, thus less patients would require antiretroviral therapy and B*5701 typing. Considering the usefulness of B*5701 screening, its prevalence in the Chilean general population,and the availability of a validated method,we conclude that HLA-B*5701 typing in Chilean HIV-infected patients about to initiate abacavir treatment is strongly recommended.

ABSTRACT

Objetivo: Describir y caracterizar la epidemiología de las consultas por intoxicaciones al Centro de Información Toxicológica de la Universidad Católica (CITUC) y conocer las variaciones epidemiológicas producidas entre los años 2004 y 2007. Método: Se realizó un estudio retrospectivo, observacional y descriptivo de las llamadas registradas por el Centro durante el periodo del año 2007. Se analizaron las siguientes variables: número total de llamadas, lugar de las llamadas, circunstancias de la exposición, tipo y números de agentes involucrados, vías de exposición, tiempo desde la exposición, características demográficas y las indicaciones y tratamientos recomendados. Resultados: Se registraron 30.171 llamadas. El 45 por ciento de las consultas provenían del hogar y el 52,5 por ciento de las instalaciones de salud. El 67,2 por ciento de las consultas fueron exposiciones accidentales y el 29,2 por ciento fueron intencionales. Los Medicamentos se presentaron en un 57,1 por ciento de las llamadas, seguidos por los Productos de uso doméstico (16,5 por ciento), Productos de uso industrial (4,7 por ciento), y Pesticidas (5,8 por ciento). La ingestión fue la vía de exposición que se predominó en un 76,6 por ciento del total de casos. El 23,3 por ciento del total de las llamadas, se realizaron dentro de la primera hora post-exposición. Las intoxicaciones en niños menores de 14 años correspondieron al 50 por ciento de las llamadas. Respecto al año 2004 aumentó la proporción de llamadas procedentes del hogar y el porcentaje de intoxicaciones intencionales. Conclusiones: Los medicamentos constituyen el principal agente de intoxicación. La principal vía de exposición es la ingesta y el principal grupo de población afectado son los niños menores de 14 años. Estos son indicadores evidentes y claros de la necesidad imperante de desarrollar campañas publicitarias preventivas e informativas, avaladas por el Ministerio de Salud, respecto al correcto almacenamiento...


Objective: To describe the epidemiological features of the intoxication cases reported to CITUC and the variations produced between 2004 and 2007. Method: A retrospective and descriptive study of the calls registered at the Center during 2007. The following data were analyzed: number and place of the calls; circunstances of exposure; type and number of agents involved; routes of exposure; time since exposure; sex and age of the subjects, and treatment and advice offered. Findings: 30.171 calls were received, 45 percent from the home and 52,5 percent from Health Care Service; 67,2 percent were accidents and 29,2 percent were intentional exposures. The agents were: medicines (57,1 percent); cleaning products (16,5 percent) pesticides (5,8 percent); other chemical and industrial products (4,7 percent). The exposure route was oral in 76,6 percent of cases; 48 percent of calls occurred within one hour of exposure. Children under six were the main group involved. Conclusion: The evidence substantiates a need for prevention campaigns, sponsored by the Health Ministry, about the correct storage of medicines and the appropiate management of poisoning at home. Child-resistant containers and education are two essential elements for prevention of pediatric intoxications.

ABSTRACT

La cocaína es la segunda droga ilegal de mayor consumo en Chile, ya sea como clorhidrato o como pasta base de cocaína. Hemos reportado que el 6 por ciento de los pacientes que ingresaron con Infarto Agudo del Miocardio con supradesnivel del ST (IAMSDST) declararon haber consumido cocaína en la semana previa al evento y la cifra real podría ser mayor. Los efectos cardiovasculares de la cocaína son causados por la inhibición de la recaptura de la noradrenalina en las terminaciones simpáticas, causando vasoespasmo, trombosis coronaria, cardiotoxicidad y aumento del consumo de O2 por taquicardia e hipertensión arterial. Los pacientes con IAM secundario al uso de cocaína son con frecuencia jóvenes que han consumido cocaína en las últimas 24 horas. El diagnóstico de isquemia o infarto en estos pacientes es difícil; el electrocardiograma puede no ser específico hasta en el 60 por ciento delos casos. Se deben utilizar troponinas como marcadores de necrosis, pues las CK-MB dan falsos positivos. El tratamiento incluye hospitalización, benzodiazepinas, morfina IV, nitratos, calcioantagonistas y oxígeno. Debe evitarse el uso de betabloqueadores, porque aumentan la vasoconstricción al liberar el tono a adrenérgico. Ante un IAMSDST, los trombolíticos deben ser usados con precaución, pues se ha reportado una mayor incidencia de hemorragia cerebral. La mejor terapia de reperfusión coronaria en estos casos es la angioplastía primaria.


Cocaine is the second most commonly used drug of abuse in Chile. We reported that 6 percent of patients with STEMI have used cocaine during the week previous to the event, number that could be higher. Cocaine inhibits catecholamine reuptake, resulting in a hyperadrenergic state, which may induce vasospasm, coronary thrombosis, cardiotoxicity, increased myocardial O2 consumption and severe hypertension. Patients with cocaine induced chest pain are frequently young men that had consumed the drug during the last 24 hours. The ECG is specific only in about 60 percent of cases. Troponins should be used to confirm AMI; CK-MB may give false positive results. Treatment of cocaine induced chest pain includes hospitalization, benzodiazepines, nitroglycerin, calcium channel blockers, IV morphine and O2. beta-blockers should not be used because they could increase vasospasm, due to unopposed alpha adrenergic tone. When STEMI is diagnosed, primary angioplasty is the preferred reperfusion method. Thrombolysis is not a first option due to increased rates of intracerebral hemorrhage in cocaine users.
